How do I become a patient?
Call to leave me a message to have a registration packet sent or register on line. Once you are registered you can call and make your first appointment.
Are there any age limitations on becoming your patient?
No. I see all ages and genders.
What if I have an insurance plan you don’t accept?
There are several options. You could see if you could switch plans. Another option is to become a patient and pay the monthly fee. When you are seen in the office I can generate a bill that you can submit to your insurance for reimbursement. I can then credit your payment for that visit to future monthly fees.
Are there any people who cannot register as your patient?
Yes, if you belong to an HMO plan I do not accept then I am not allowed to provide medical services to you. An HMO requires you to sign up with a primary care provider on their panel, and that is the only practice you can go to for primary care.
